No Cost to Eligible Participants
The Lifestyle Change Program is offered at no cost to eligible residents.
Insurance information is collected only after enrollment to bill insurance providers when applicable. Participants are not responsible for out-of-pocket costs.
Residents of Bonsall, De Luz, Fallbrook, and Rainbow are eligible regardless of insurance status.

- Manage stress.
- Build healthy habits that last.
- Stay motivated with support from your Lifestyle Coach and classmates.

The Lifestyle Change Program is a one-year journey that begins with weekly group sessions to help you build healthy habits. As you progress, meetings become less frequent, while support and accountability continue throughout the year.
