You will get a vaccination card that lists which COVID-19 vaccine you received, the date you received it, and where you received it. Keep your vaccination card in case you need it for future use. Please note, on the back of the card it states:
You can report possible adverse reactions following COVID-19 vaccination to the Vaccine Adverse Event Reporting System (VAERS) at vaers.hhs.gov.
Consider taking a picture of your vaccination card as a backup copy.
The California Department of Public Health has an online process where you can access and download a digital copy of your COVID-19 vaccination history.
